When you hear about gout, the reactions from those who have experienced it and those who haven't are completely different.
Those who haven't had it might think, "Isn't it just a little pain in the toe?" But someone who has had a severe attack says they never want to go through it again.
Gout is an inflammatory arthritis that occurs when there is too much uric acid in the blood.
When a substance called purine is broken down in our body, uric acid is produced. If the kidneys cannot excrete this uric acid sufficiently or if too much is produced, the concentration of uric acid in the blood increases.
The problem is that when uric acid levels rise above a certain point, it can form tiny needle-like crystals in the joints.
The immune system recognizes these crystals as foreign substances and attacks them, causing sudden and intense inflammation. This is a gout attack.
The most well-known area affected is the joint of the big toe. There are cases where someone wakes up in the middle of the night because their big toe hurts so much.
The joint becomes red, swollen, and hot, and the pain can be so severe that even the touch of a sock or a blanket resting lightly on the foot is unbearable.
What makes it even more troublesome is that walking becomes difficult. It may seem like just one toe is hurting, but every time weight is put on it, pain occurs, making even going to the bathroom a challenge.
Gout doesn't only occur in the big toe. It can also affect other joints like the ankle, top of the foot, knee, wrist, fingers, and elbow.
The really annoying thing about gout is that it may not just be a one-time painful experience. Initially, the attacks may improve after a few days, and there may be months or even years without any issues.
However, if uric acid levels remain high in the blood, crystals can continue to accumulate. Over time, the intervals between attacks may shorten, and problems can arise in multiple joints.
If left untreated for a long time, uric acid crystals can form lumps known as tophi. This can cause the fingers, elbows, and areas around the toes to become bumpy and lead to joint damage.
The kidneys should not be ignored either. Since uric acid is excreted through the kidneys, gout and kidney function are closely related.
In some patients, uric acid-related kidney stones can also develop.
As a result, habits such as drinking a lot of alcohol, especially beer and spirits, or consuming excessive amounts of red meat, organ meats, and certain seafood can increase the risk of gout or trigger attacks.
Regularly consuming sugary drinks, especially those high in fructose, is also not advisable. Dehydration can also be a factor that triggers attacks.
However, it is important not to think of gout simply as a disease caused by eating too much meat. Various factors such as genetics, kidney function, obesity, metabolic diseases, and medications can all play a role.
No matter how well one manages their diet, some people may still not be able to lower their uric acid levels sufficiently.
Therefore, for those who experience recurrent gout attacks, it is important to consult a doctor to manage blood uric acid levels rather than just trying to endure by being careful with food.
If necessary, medications to lower uric acid may be taken for an extended period. These medications are different from pain relievers that are taken only when experiencing pain.
Ultimately, the reason gout is frightening is not just because of the pain. Once an attack starts, it can be painful enough to disrupt daily life, and if left untreated, it can return, leading to problems with joints and kidneys if not managed over the long term.On the other hand, if properly diagnosed and uric acid levels are consistently managed, many patients can live without experiencing attacks.
Gout should not be seen as a disease that one must simply endure; rather, it is a condition that requires proper management from the start to avoid suffering.
